Monday, Tuesday, Wednesday, & Friday 9am-1pm

Thursday 9am-1pm & 4pm-7pm

MUST HAVE THE FOLLOWING WITH YOU:

  • Valid Photo ID for all adults in household
  • Social Security Cards for parents and children
  • Birth Certificates on all children (can also use Medicaid card w/ DOB; shot records w/ DOB, or school records w/ DOB)
  • Proof of Income (TANF, Food Stamps, Disability, Employment, Unemployment, Child Support)
  • Proof of Rent
  • Proof of Address
  • Official custody or guardianship paper if applicant is not the biological parent
  • Sizes for children through age 16
